Dr. Jacob Chapman, D.C.

Dr. Chapman of Align Chiropractic graduated from Northwestern Health Science University in 2009. After completing an internship at the Brooks Chiropractic & Wellness Clinic, he accepted a full-time position at the clinic. Dr. Chapman is Webster certified.

In January 2015, Dr. Chapman purchased Brooks Chiropractic and was excited to grow with the practice in Port Washington. He renamed the practice to Align Chiropractic hoping to make the practice feel like family. That's why "Aligning Our Family with Yours" is our mantra.

He is dedicated to patients and the quest to improve their health through natural health care, believing that all patients, including children, deserve to be cared for as individuals. He is a member of the Chiropractic Society of Wisconsin, Holistic Pediatric Association, and the International Chiropractic Pediatric Association.

Dr. Chapman, his wife, and their three sons and daughter reside in Fredonia, WI. He enjoys time outdoors with his family.

Kim Dean, Chiropractic Technician

Kim joined our clinic in May 2011. She resides in Grafton with her husband where they have raised three children. After several years in the retail management field, along with managing a fitness and wellness center, she decided to follow her passion for helping others in the field of health and wellness. Kim is a solid believer in chiropractic care and in the well-being of individuals and their health.

Christin Herzfeld, Chiropractic Technician 

My name is Christin, but everyone calls me Chris. I joined Align in July 2023. 

I live in Port Washington with my husband, Mike, and our small family of pets. We have two cats and two Siberian Huskies. I enjoy running, hiking, paddle boarding, and rock climbing. Tough Mudder is my favorite endurance activity, and chiropractic care allows me to maintain my health and ability to continue participating. During the winter the dogs and I enjoy sledding and skijoring. After a long day, I enjoy sitting with a warm beverage, a good book, and a cat on my lap.

Ashlyn Chmielewski, Chiropractic Technician

Our energenic Ashlyn joined the Align Chiropractic family in July 2024.  When she's not busy spoiling her two cats or enjoying time with her family and friends, she is all about helping people feel their best.  She loves chiropractic care, " because lets face it, we've only got one body to live with".  At Align Chiropractic, Ashlyn appreciates that we welcome everyone with open arms--just like family.

Amber Large, Chiropractic Administrative Assistant

Amber has been working at the clinic since 2006 while in college. After graduating, Amber has worked at the clinic part-time on Saturdays. She enjoys the relationships she has formed here and enjoys spending time with her dog Nash.
